Wisconsin title forms, explained
WisDOT has a separate form for most title situations, and an application with the wrong or missing paperwork takes longer. Find your situation, read what the form is for, then get the form itself free from WisDOT.
TitleFiled is not WisDOT. Every official form is free at wisconsindot.gov.
No title or a problem title
MV2082: Application for Bonded Certificate of Title
For a vehicle you own but have no title to send. Mailed with an MV1 and your evidence.
MV2083: Application for Certificate of Title for Light Trailer
Light trailers under 3,000 lb. The form says it is for Wisconsin boat and trailer dealers only.
Buying and selling
MV1: Wisconsin Title and License Plate Application
The main title application. Almost every other form on this page travels with one.
MV2488: Vehicle Transfer and Odometer Mileage Statement
Odometer statement some transfers need. Not downloadable; DMV service centers have it.
MV2928: Instructions for Selling a Vehicle (with Sample Bill of Sale)
WisDOT's one-page seller checklist plus a sample bill of sale. Optional, never mailed in, and it does not replace the title.
Inherited vehicles
Moves a Wisconsin-titled vehicle out of a person's name after they die, without probate papers in most family cases.
Lien sales and abandoned vehicles
MV2419: Abandoned / Unregistered Vehicle Transfer Certificate
Comes from the town or police when they sell an abandoned car. You do not fill one out yourself.
MV2881: Involuntary Lien Transfers and Abandonment of Property to a Landlord
Filled out by a repair shop, tow company, storage operator or landlord that sold a car to collect what it was owed. The buyer titles with it.
MV2882: Abandonment of Property to a Landlord
For a Wisconsin landlord whose tenant moved out and left a titled vehicle behind. The landlord sells or keeps it and the new owner titles with this form.
Salvage and rebuilt vehicles
MV2459: Junked Vehicle Bill of Sale
Moves a junk vehicle to a salvage dealer. No title is ever issued on it.
MV2673: Major Parts Statement (Repaired Salvage Vehicle or Homemade)
Lists every major part you replaced on a salvage or homemade vehicle, with a bill of sale for each. Handed to the inspector, not mailed.
The seller of a frame from a junked vehicle vouches it is sound. Used with a rebuilt salvage inspection.
Fixing or picking up a title
MV1020: Title/Registration Correction Request
Fixes a mistake on a title, registration or Confirmation of Ownership WisDOT already issued: a misspelled name, wrong VIN digit, wrong color.
MV2119: Replacement Title Application
Lost, stolen or damaged title that is already in your name. $20.
Explains a change or erasure in the buyer, lien or odometer area of a title.
MV2932: Permission to Pick Up Title
Lets the owner name someone else, or a dealer, to collect a title from WisDOT. ID is required.
