Vehicle history worksheet for a bonded title

WisDOT's bonded title form, MV2082, asks for a written statement of how you got the vehicle. This worksheet helps you get the facts straight first, then prints them on one page.

This organizes your facts. You write and sign your own statement.

How it works

Answer in your own words. Skip anything you do not know yet; it prints as a blank line you can fill in by hand. Your fact sheet builds at the bottom as you type, word for word. Nothing is rewritten.

Nothing you type into the worksheet leaves this page. It is not saved and not sent anywhere. Refreshing or closing the tab clears it, so print or save it as a PDF when you are done.

When you are ready, write your statement on page 3 of MV2082 yourself. The form's instructions list five points: how you got the vehicle, who from, how you came to have it without a title and what you did to get one, how long you have had it, and any history you know of. You sign the form certifying the information is true.
